    Registered Nurse (RN) Home Hospice | Southeast Suburbs (Blue Team)

    About the Role

    We are seeking a compassionate and experienced Home Hospice Registered Nurse (RN) to join our Southeast Suburbs Blue Team. This role involves providing high-quality, patient-centered end-of-life care in patients homes, focusing on comfort, dignity, and support for both patients and their families.


    Facility & Work Setting

    • Home Hospice position (field-based patient homes)
    • Initial onboarding and supply pickup at regional office in Oak Brook
    • Quarterly in-person team meetings at Trinity Hospital

    Patient Population

    • Adult patients only
    • Common diagnoses include:
      • Advanced cancer
      • End-stage COPD
      • Failure to thrive
      • Terminal illnesses and end-stage disease processes

    Key Responsibilities

    • Deliver comprehensive hospice nursing care in a home setting
    • Perform wound care, dressing changes, and wound VAC management
    • Monitor and document wound measurements and progress (weekly)
    • Manage drains (Pleurex, nephrostomy, Foley catheter)
    • Provide end-of-life care planning and symptom management
    • Conduct patient and family education
    • Coordinate care with interdisciplinary teams
    • Perform triage and respond to patient needs

    Required Qualifications

    • Minimum 3 years of Home Hospice or Home Health experience
    • Active RN license
    • BLS certification
    • Valid driver s license, auto insurance, and reliable vehicle

    Preferred Skills

    • Strong organizational and time management skills
    • Ability to work independently in a field setting
    • Excellent communication and patient care skills

    Caseload

    • 12 15 patients

    Technology & Support

    • EMR: EPIC Remote Client
    • Company-provided phone, laptop, and trunk stock
    • Ongoing support from hospice RNs and management (available by phone)

    Coverage Area

    Includes (but not limited to):

    • Lynwood
    • Calumet
    • Calumet City
    • Calumet Park
    • Riverdale
    • Dolton

    Schedule & Work Hours

    • Monday Friday | 8:00 AM 4:30 PM
    • No weekends
    • No on-call
    • No holiday requirements
    • Set schedule

    Orientation

    • New Employee Orientation (NEO)
    • 1 2 weeks (based on experience and comfort level)
