o Address: Servicing North Milwaukee and the counties of Washington, Ozaukee, Waukesha, and Jefferson
General Information:
o Beds: N/A
o Will you accept a traveler with 1-year experience? 2 years of Hospice experience needed
o Will you accept a first-time traveler? Yes, if they have Hospice and EPIC Remote Client experience
Patient Types:
o Adults only End of Life patients
Patient Ratios:
o 1:1
Required Certifications:
o BLS
o Valid DL and insurance
Skills required:
o End of life care/planning, wound assessments, care coordination, pain/symptom management, triage
Support on the Unit:
o RNs
o Resource Nurse
o Case Managers
Technology/Equipment:
o EMR: EPIC Remote Client experience preferred
o IV pumps: Sapphire
Floating: If yes, where?
o Anywhere within the counties
Orientation (how many shifts):
o up to 1 week, based on comfort level
Scheduling:
o Weekend rotation: No weekends
o Call? No call
o Holiday Expectations: No holidays
o Approval of time off: Can we approve time off on your behalf, or would you like all requests sent to you? Manager to approve all RTO
o Shift times: M-F 0800-1630; (4) 10 hour shifts may also be available
