Job Details- Evaluates patients referred for Hospice and /or Palliative Care services. Manages and facilitates the transition of patients
- in hospice or palliative care in accordance with applicable law, regulations and agency policies and procedures. Provides
- clinical support, leadership and oversight for growth and development of community and facility based hospice programs
- within the Metropolitan Jewish Hospice. Duties include planning, delivering customer service, development for service
- area expansion, coordination of internal and external clinical marketing efforts, community outreach, education, and
- clinical service/support for hospice patient/family direct care.
- Responds to all requests and all referrals from Discharge Planners and assists with obtaining the
- Election of Benefits & admissions.
- Evaluates for Hospice Eligibility following the Hospice Conditions of Participation according to
- the state and federal regulations.
- Gathers all information required by the agency to initiate a referral and faxes the information to the
- hospice intake unit.
- Makes rounds on all units of the hospital on a minimum of daily and consistent basis.
- Provides physician, referral sources (including the discharge planners,) admissions department and
- the medical facilities with education regarding Hospice and Palliative care eligibility and end of life care
- Works in conjunction with discharge planning staff to identify problems that may impede the
- transition of the patient to hospice or palliative care and assist with resolution as needed.
- Communicates and interprets agency admission criteria, reimbursement criteria and policies to
- patient/significant other, members of the interdisciplinary health care team and other personnel as indicated.
- Responsible for the daily coordination of Hospice care on the Hospice Inpatient Unit (IPU).
Qualifications:
A minimum of two years’ experience in either Medical-Surgical or, Community
Health. Hospice or Oncology nursing preferred.
