Description

Position Overview

Seeking an experienced and compassionate Home Health Registered Nurse (RN) to provide skilled nursing services throughout Charles, Calvert, and St. Mary's counties. This field-based position is responsible for conducting admissions, follow-up visits, and discharges while delivering high-quality patient-centered care in the home setting. The ideal candidate will have HomeCare HomeBase (HCHB) experience, strong clinical skills, and the ability to work independently while coordinating care with interdisciplinary team members.

Schedule & Work Details

  • Location: Charles, Calvert, and St. Mary's Counties, Maryland
  • Schedule: Monday through Friday, 8:00 AM - 4:30 PM
  • Weekly Hours: 40 hours per week
  • Pay Rate: $51 - $53 per hour
  • Productivity Expectation: 30 points per week
  • On-Call/Weekend Requirement: None
  • Mileage Reimbursement: Yes, reimbursed between patient visits
  • Dress Code: Navy blue scrubs

Key Responsibilities

  • Perform home health admissions, recertifications, follow-up skilled nursing visits, and discharge visits.
  • Complete comprehensive patient assessments and develop individualized plans of care.
  • Monitor patient conditions and provide skilled nursing interventions according to physician orders and agency policies.
  • Administer medications, treatments, wound care, and other nursing services as needed.
  • Educate patients and caregivers regarding disease processes, medications, treatments, and self-management strategies.
  • Accurately document all patient encounters, assessments, and interventions in HomeCare HomeBase (HCHB).
  • Collaborate with physicians, therapists, case managers, and other healthcare professionals to coordinate patient care.
  • Ensure compliance with all agency, state, federal, and regulatory requirements.
  • Maintain timely and accurate completion of clinical documentation.

Required Qualifications

  • Active Registered Nurse (RN) license.
  • Current CPR certification.
  • Previous home health nursing experience.
  • Experience using HomeCare HomeBase (HCHB).
  • Strong clinical assessment and patient care skills.
  • Ability to work independently and effectively manage a field-based caseload.
  • Valid driver's license, reliable transportation, and ability to travel throughout the assigned territory.

Preferred Qualifications

  • Experience with admissions, recertifications, and discharge visits in a home health setting.
  • Strong time management and organizational skills.
  • Excellent communication and patient education abilities.
  • Knowledge of Medicare home health regulations and documentation requirements.
  • Experience coordinating care within an interdisciplinary team environment.
