Job Details:

  • Schedule: Monday-Friday, 8:00 AM - 4:30 PM
  • Duration: 14-week contract
  • Mileage Reimbursement: Yes, between patient visits
  • Location: Renton, WA
  • Job Type: Contract
  • Pay Range: $53-$56/hour

Pay offered is based on experience, expertise, credentialing, and education.

Duties:

  • Provide skilled nursing care to patients in their homes as prescribed by the physician.
  • Develop, implement, and revise individualized care plans in collaboration with physicians and other healthcare professionals.
  • Complete comprehensive patient assessments, including OASIS assessments and medication reconciliation.
  • Coordinate care and communicate effectively with physicians, patients, families, and interdisciplinary team members.
  • Supervise and provide direction to Home Health Aides and LPN/LVNs in accordance with state and federal regulations.
  • Maintain accurate, timely, and thorough documentation of all patient visits and care provided.
  • Participate in care coordination, case conferences, quality improvement initiatives, and patient outcome management.
  • Assist with orientation and training of new staff and serve as a preceptor when needed.
  • Participate in weekend rotations and provide flexibility to meet patient care needs.

Requirements:

  • Active RN License in the state of practice
  • Current CPR Certification
  • Current Driver's License, vehicle insurance, and reliable transportation
  • Ability to complete OASIS assessments and manage patient care independently

Preferred Qualifications:

  • Home Health experience preferred
  • Experience working in a point-of-care setting preferred
