Position Overview
Seeking a dedicated and compassionate Home Health Registered Nurse (RN) to provide skilled nursing care to patients in their homes throughout the assigned Indiana service area. The RN will perform comprehensive assessments, deliver hands-on clinical care, educate patients and caregivers, and coordinate services to promote positive patient outcomes. This role requires strong clinical skills, independence, and the ability to manage a caseload across multiple counties.
Schedule & Work Details
- Coverage Areas: New Albany, IN
- Schedule: Full-time
- Shift: Monday through Friday, 8:00 AM - 4:30 PM
- On-Call Requirement: 1-2 days per week
- Weekend Requirement: One weekend per month
- Mileage Reimbursement: Yes
- Pay Rate:$52.00 - $54.00 per hour
Key Responsibilities
- Complete comprehensive OASIS assessments, including Start of Care, Recertification, Resumption of Care, Transfer, and Discharge assessments.
- Provide skilled nursing services to patients in their homes in accordance with physician orders and individualized care plans.
- Perform wound assessments, wound care treatments, and ongoing monitoring of wound healing progress.
- Manage urinary catheter care, maintenance, troubleshooting, and patient/caregiver education.
- Collect laboratory specimens and monitor diagnostic results as appropriate.
- Provide disease process education to patients and caregivers to support self-management and improved health outcomes.
- Deliver medication education, reconciliation, and ongoing medication management.
- Perform PICC line care, maintenance, monitoring, and patient/caregiver education.
- Supervise and conduct required visits for Licensed Practical Nurses (LPNs) and Home Health Aides.
- Collaborate with physicians, therapists, social workers, and interdisciplinary team members to coordinate patient care.
- Participate in team case conferences and care coordination meetings.
- Monitor patient progress and communicate changes in condition to the appropriate healthcare providers.
- Maintain accurate, timely, and compliant clinical documentation.
- Ensure adherence to Medicare regulations, agency policies, and professional nursing standards.
Required Qualifications
- Active, unrestricted Registered Nurse (RN) license.
- Current CPR certification.
- Minimum of one year of RN experience.
- Valid driver's license, reliable transportation, and the ability to travel throughout the assigned coverage area.
- Strong assessment, communication, and organizational skills.
- Ability to work independently in a field-based setting.
Preferred Qualifications
- Previous home health nursing experience.
- Experience using Homecare Homebase (HCHB).
- Strong knowledge of OASIS documentation and Medicare home health regulations.
- Experience managing a home health patient caseload.
- Excellent patient education and care coordination skills.
- Ability to build strong relationships with patients, families, and interdisciplinary team members.
