Position Overview

The Registered Nurse (RN) provides comprehensive, patient-focused care in a home health setting. This role is responsible for assessing patient needs, developing and managing individualized care plans, and collaborating with physicians and interdisciplinary teams to ensure high quality outcomes. The RN also ensures compliance with all regulatory, clinical, and documentation standards while supporting patient safety and continuity of care.

Schedule & Work Details

  • Coverage Area: King County and Snohomish County, WA
  • Schedule: Monday through Friday, 8:00 AM to 4:30 PM
  • On Call Requirement: Yes, includes 5 weekdays and 2 weekends minimum
  • Pay Rate: $51 to $53 per hour
  • Mileage Reimbursement: Yes, between patient visits from first to last visit
  • Productivity Expectation: Minimum of 30 productivity points per week
  • Flexibility: Required for weekend rotation and potential holiday coverage

Key Responsibilities

Clinical Competence

  • Initiate, develop, implement, and revise individualized plans of care in collaboration with physicians and interdisciplinary teams
  • Perform initial and ongoing nursing assessments to determine patient eligibility and medical necessity
  • Supervise and direct care provided by home health aides and LPNs, including task delegation and required supervisory visits in compliance with regulations

Documentation and Care Delivery

  • Provide skilled nursing care in accordance with the plan of care and infection control standards
  • Complete OASIS assessments accurately and timely, including medication reconciliation
  • Maintain accurate, thorough, and timely documentation in accordance with agency and payer requirements
  • Ensure all documentation is synced promptly per agency policy

Quality and Care Coordination

  • Conduct initial and comprehensive evaluations, ensuring compliance with payer and state-specific guidelines
  • Develop and coordinate care plans with physicians and care teams
  • Communicate effectively with internal staff regarding patient needs, scheduling, and care delivery updates
  • Coordinate care with clinicians, physicians, patients, and families to ensure continuity and quality outcomes
  • Participate in efforts to reduce hospitalizations and improve patient safety and outcomes

Professionalism and Teamwork

  • Maintain a positive attitude, professionalism, and willingness to support patients and team members
  • Attend and actively participate in meetings, including case conferences and training sessions
  • Follow direction from leadership and complete assigned tasks promptly
  • Support onboarding and training of new staff and serve as a preceptor when needed
  • Participate in survey readiness, quality improvement initiatives, and utilization management processes
  • Maintain schedule flexibility to meet patient care demands

Required Qualifications

  • Active, unencumbered Registered Nurse license in Washington State or compact multi-state license
  • Current Basic Life Support (BLS) certification
  • Strong clinical assessment and care planning skills
  • Ability to work independently and collaboratively in a home health environment

Preferred Qualifications

  • Prior home health experience
  • Experience with HCHB or PointCare systems
  • Strong communication, organization, and time management skills
  • Experience completing OASIS documentation
