Position Overview
Seeking an experienced and autonomous Registered Nurse (RN) to provide skilled home health services throughout Lexington and the surrounding service area. This position is responsible for delivering high-quality nursing care, completing comprehensive assessments and OASIS documentation, coordinating care across the interdisciplinary team, and promoting positive patient outcomes. The ideal candidate will have prior home health experience, strong OASIS knowledge, and proficiency with Homecare Homebase (HCHB).
Schedule & Work Details
Agency: Commonwealth Home Health - Lexington (KY118)
Coverage Areas: Bourbon, Clark, Estill, Fayette, Franklin, Harrison, Jessamine, Nicholas, and Woodford Counties
High-need coverage areas include Estill, Clark, and Nicholas Counties
Schedule: Monday through Friday, 8:00 AM to 4:30 PM
Weekly Hour Guarantee: 32+ hours
Pay Rate: $51.00 - $53.00 per hour
Productivity Expectation: 30 points per week
On-Call Requirement:
Approximately 1 weekend every 4 weeks
5-6 weekday on-call shifts per month
Mileage Reimbursement: Reimbursed between patient visits only; travel from home to the first patient and from the last patient to home is not reimbursed
Required Meetings:
Daily morning Teams call at 8:30 AM
Weekly case conference every Wednesday at 8:30 AM
