Position Overview

Seeking a dedicated and compassionate Home Health Registered Nurse (RN) to provide skilled nursing care to patients throughout Lee County, Florida. The RN will perform comprehensive patient assessments, complete OASIS documentation, deliver skilled nursing interventions, and coordinate care to help patients achieve the highest level of independence and wellness in their homes. This role requires strong clinical judgment, excellent communication skills, and the ability to manage an independent field-based caseload.

Schedule & Work Details

  • Coverage Area: Lee County, FL
  • Schedule: Full-time, 40 hours per week
  • Shift: Monday through Friday, 8:00 AM - 5:00 PM
  • On-Call/Weekend Requirement: As needed
  • Mileage Reimbursement: Yes
  • Patient Care Setting: Home Health
  • Pay Rate: $51.00 - $53.00 per hour

Key Responsibilities

  • Provide skilled nursing visits to patients in their homes in accordance with physician orders and established care plans.

  • Complete OASIS assessments, including:

  • Start of Care (SOC)

  • Resumption of Care (ROC)

  • Recertification Assessments

  • Discharge Assessments

  • Conduct comprehensive physical assessments and ongoing evaluation of patient status.

  • Deliver skilled nursing treatments and interventions, including wound care, ostomy care, Foley catheter care, and other prescribed services.

  • Monitor patient conditions and communicate significant changes to physicians and interdisciplinary team members.

  • Educate patients and caregivers on disease processes, medication management, treatment plans, safety precautions, and self-care techniques.

  • Coordinate care with physicians, therapists, home health aides, and other healthcare professionals.

  • Develop, implement, and update individualized care plans based on patient needs and clinical findings.

  • Maintain timely, accurate, and compliant clinical documentation.

  • Ensure compliance with Medicare regulations, agency policies, and professional nursing standards.

  • Participate in case conferences and interdisciplinary team meetings as needed.

  • Manage a daily travel schedule while providing exceptional patient-centered care throughout the assigned coverage area.

Required Qualifications

  • Active and unrestricted Registered Nurse (RN) license in the State of Florida.
  • Ability to independently perform skilled nursing visits and patient assessments.
  • Valid driver's license, reliable transportation, and ability to travel throughout Lee County.
  • Strong clinical assessment and critical thinking skills.
  • Excellent communication, documentation, and organizational abilities.
  • Ability to work independently in a field-based healthcare environment.

Preferred Qualifications

  • Previous home health experience.
  • Experience completing OASIS assessments and documentation.
  • Knowledge of Medicare home health regulations and standards.
  • Strong patient education and care coordination skills.
  • Professional, well-spoken communication style.
  • Experience managing an independent patient caseload in a home health setting.
