Description

Position Overview

Seeking a compassionate and experienced Home Health Registered Nurse (RN) to provide skilled nursing care to patients throughout Collier County. The RN will be responsible for completing comprehensive patient assessments, delivering skilled nursing services in the home, coordinating care with the interdisciplinary team, and ensuring accurate and compliant clinical documentation. This role is ideal for a nurse who enjoys working independently while making a meaningful impact on patients' health and quality of life in the home setting.

Schedule & Work Details

  • Coverage Area: Collier County, FL
  • Schedule: Full-time, 40 hours per week
  • Shift: Monday through Friday, 8:00 AM - 5:00 PM
  • On-Call/Weekend Requirement: As needed
  • Mileage Reimbursement: Yes
  • Patient Care Setting: Patient homes throughout the service area
  • Pay Rate: $51.00 - $53.00 per hour

Key Responsibilities

  • Provide skilled nursing visits to patients in their homes in accordance with physician orders and individualized care plans.

  • Complete OASIS assessments, including:

  • Start of Care (SOC)

  • Resumption of Care (ROC)

  • Recertification Visits

  • Discharge Assessments

  • Perform comprehensive head-to-toe assessments and ongoing evaluations of patient status.

  • Deliver skilled nursing interventions including wound care, ostomy care, Foley catheter care, and other clinical treatments as ordered.

  • Monitor patient progress and identify changes in condition requiring physician notification or care plan modification.

  • Educate patients and caregivers regarding disease management, medications, treatment plans, and home safety.

  • Coordinate care with physicians, therapists, aides, and other members of the healthcare team.

  • Maintain timely, accurate, and compliant documentation within the electronic medical record system.

  • Ensure adherence to Medicare, state, and agency regulations and standards.

  • Participate in case conferences and interdisciplinary care planning activities.

  • Manage a daily patient schedule while traveling throughout the assigned coverage area.

Required Qualifications

  • Active Registered Nurse (RN) license in the State of Florida.
  • Ability to perform comprehensive assessments and skilled nursing visits independently.
  • Valid driver's license, reliable transportation, and ability to travel throughout Collier County.
  • Strong clinical assessment, organizational, and time-management skills.
  • Excellent verbal and written communication skills.

Preferred Qualifications

  • Previous home health experience.
  • Experience completing OASIS assessments and documentation.
  • Knowledge of Medicare home health regulations and compliance requirements.
  • Strong patient education and care coordination abilities.
  • Excellent communication skills and professional bedside manner.
  • Ability to work independently and effectively manage a field-based caseload.
