Position Overview
Seeking an experienced and dedicated Home Health Registered Nurse (RN) to provide exceptional patient care throughout Richland County and surrounding counties. This field-based position is responsible for managing an assigned patient caseload, coordinating care, performing skilled nursing services, and ensuring patients receive high-quality care in the comfort of their homes. The ideal candidate will have prior home health experience and strong case management skills.
Schedule & Work Details
- Coverage Area: Richland County and surrounding counties
- Schedule: Monday through Friday, 8:00 AM to 4:30 PM
- Weekly Hours: 40 hours
- On-Call Requirement: One weekday on-call rotation per month
- Weekend Requirement: One weekend on-call rotation per month
- Pay Rate:$51.00 - $53.00 per hour
- Mileage Reimbursement: Yes, reimbursed between patient visits (from first patient to last patient)
Key Responsibilities
- Manage and coordinate care for an assigned home health patient caseload.
- Perform comprehensive patient assessments and ongoing evaluations.
- Develop, implement, and update individualized care plans based on patient needs and physician orders.
- Provide skilled nursing services in the home setting.
- Monitor patient progress and communicate changes in condition to physicians and interdisciplinary team members.
- Educate patients and caregivers on disease management, medications, treatment plans, and home safety.
- Coordinate care with physicians, therapists, social workers, and other healthcare professionals.
- Ensure timely, accurate, and compliant clinical documentation.
- Maintain productivity standards while delivering high-quality, patient-centered care.
- Participate in required on-call rotations and provide support as needed.
Required Qualifications
- Active Ohio Registered Nurse (RN) license in good standing.
- Previous home health nursing experience required.
- Strong case management and clinical assessment skills.
- Ability to work independently and effectively manage a field-based schedule.
- Excellent communication, organizational, and time management abilities.
- Valid driver's license and reliable transportation.
Preferred Qualifications
- Experience with Homecare Homebase (HCHB) documentation.
- Knowledge of home health regulations, standards, and best practices.
- Strong critical thinking and problem-solving skills.
- Ability to build positive relationships with patients, families, and interdisciplinary team members.
- Experience managing multiple patients while meeting productivity expectations.
