Position Overview
Seeking a compassionate and skilled Registered Nurse (RN) to provide high-quality patient care in the home health setting. The RN will be responsible for delivering direct patient care, completing comprehensive assessments, developing and implementing care plans, and coordinating services to support patients in achieving optimal health outcomes. This role requires strong clinical skills, independence, and experience with OASIS documentation.
Schedule & Work Details
- Location: Baton Rouge, LA
- Schedule: Full-time, 40 hours per week

- Pay Rate:$51.00 - $53.00 per hour
Key Responsibilities
- Provide skilled nursing care to patients in their homes in accordance with physician orders and agency policies.
- Complete comprehensive patient assessments, including OASIS documentation.
- Develop, implement, and update individualized patient care plans.
- Perform wound assessments and provide wound care treatments as ordered.
- Administer and monitor IV therapies and other prescribed treatments.
- Conduct medication management, reconciliation, education, and monitoring.
- Educate patients and caregivers on disease processes, medications, treatments, and safety measures.
- Monitor patient progress and communicate changes in condition to physicians and interdisciplinary team members.
- Coordinate care with therapists, physicians, social workers, and other healthcare professionals.
- Maintain accurate, timely, and compliant clinical documentation.
- Participate in on-call and weekend coverage as assigned.
Required Qualifications
- Active and unrestricted Registered Nurse (RN) license.
- Minimum of one year of nursing experience.
- Home health experience required.
- Experience completing OASIS assessments and documentation.
- Experience with wound care, IV therapy, and medication management.
- Current BLS certification.
- Valid driver's license, reliable transportation, and proof of auto insurance.
- Ability to travel throughout the assigned coverage area.
Preferred Qualifications
- Previous experience in Medicare-certified home health.
- Strong knowledge of home health regulations and compliance standards.
- Excellent assessment, communication, and organizational skills.
- Experience working independently in a field-based clinical setting.
