Position Overview
We are seeking an experienced RN to coordinate care for members with complex medical needs, including G-tubes and enteral feeding services. The role is primarily remote with periodic in-person assessments throughout the Phoenix area.
Required Qualifications
Current Arizona Registered Nurse (RN) license.
4–5 years of recent RN experience in home health, hospice, long-term care, nursing case management, or complex care coordination.
Recent hands-on experience with medical feeding tubes, including G-tubes.
Minimum 2 years of experience working with individuals with disabilities.
Valid Arizona driver's license and ability to travel for in-person assessments.
Valid Arizona Fingerprint Clearance Card.
Proficiency with Microsoft Word, Google Workspace, and Adobe Acrobat.
Strong clinical assessment and documentation skills.
Excellent verbal and written communication skills.
Strong organizational and case-management skills.
Ability to work independently in a hybrid/remote environment.
Strong collaboration skills with multidisciplinary teams and healthcare/service providers.
Responsibilities
Complete comprehensive nursing assessments and reports according to applicable requirements and policies.
Document medical necessity for skilled nursing and other services.
Conduct in-person assessments every 90 days and as needed.
Assess members with G-tubes, enteral feeding needs, and complex medical conditions.
Review medical records, care plans, and supporting documentation.
Coordinate nursing referrals and communicate with providers, agencies, vendors, and service partners.
Place and follow up on vendor calls for medical services and supplies.
Collaborate with members, families, caregivers, and healthcare providers.
Participate in multidisciplinary care-planning meetings.
Ensure services and recommendations comply with applicable policies and guidelines.
Maintain accurate and timely clinical documentation.
Complete required assessments, reports, referrals, and other documentation.
Identify changes in medical or functional needs and communicate concerns appropriately.
Educate members, families, and caregivers regarding healthcare needs and services.
Explain complex medical information in an understandable manner.
Support continuity of care and appropriate utilization of services.
Participate in quality-improvement initiatives and special projects.
Attend required meetings, training, and professional-development activities.
Maintain confidentiality and protect member health information.
