    The RN Director of Case Management leads with executive presence, builds trust across the department, and develops a team of Case Managers and Social Workers that performs. The Director communicates and presents with the polish physicians and senior administration expect and delivers a consistent record of results against goals.

    The Director thrives as a collaborator, influencing well beyond a direct reporting line, managing multiple priorities, adapting quickly to change, and navigating conflict with tact and diplomacy.

    Qualifications:

    • Registered Nurse (RN) required
    • Bachelor of Science in Nursing (BSN) required
    • Master’s Degree in a health-related field required
    • Director of Case Management experience in an Acute Hospital Setting required
    • Certification in Case Management (CCM/ACM) preferred

    Responsibilities:

    • Provides strategic leadership and operational oversight for Case Management across the acute care continuum, including the Case Management model, staffing skill mix, and daily department operations
    • Directs the clinical Case Managers and Social Workers, developing the team and holding accountability for individual and departmental performance
    • Builds and implements the Case Management process along with the tools and systems that make multidisciplinary Care Coordination effective and efficient
    • Maintains accountability for quality patient care outcomes alongside appropriate Utilization Management and Resource Utilization across the continuum
    • Partners with physicians, market and system leadership, and community organizations to identify opportunities that improve patient care and services
    • Reports within a dual matrixed structure shared between hospital and system leadership
    • Oversees departmental performance through process review, data monitoring, and sustainable performance improvement activities
    • Ensures compliance with organizational policies and the requirements of regulatory agencies
