RN Coordinator

The Transplant Coordinator is responsible for coordinating and managing the care of transplant patients while waiting for a transplant. This role provides clinical expertise, patient education, care coordination, regulatory compliance, and interdisciplinary collaboration to support kidney, kidney-pancreas, and pancreas transplant programs. Essential Responsibilities Coordinate pre-transplant evaluations. Assess patient health status Serve as a communication link between physicians, patients, families and multidisciplinary team members. Maintain compliance with United Network for Organ Sharing (UNOS), OPTN, CMS, and organizational requirements. Complete transplant-related documentation, including patient listings and status changes. Collaborate with case management, financial coordinators, social workers to support patient needs. Maintain accurate and current patient records and transplant databases. Qualifications Required Current Registered Nurse (RN) license in the State of Louisiana. Bachelor of Science in Nursing (BSN) or higher. Minimum of three years of clinical nursing experience. Strong assessment, communication, organizational, and critical thinking skills. Ability to work collaboratively in a multidisciplinary healthcare environment. Preferred Transplant nursing experience. Certification in transplant nursing (CCTC, CPTC, or equivalent). Experience with UNOS/OPTN regulations and reporting requirements. Knowledge, Skills, and Abilities Knowledge of transplant patient management and regulatory requirements. Ability to coordinate complex care across multiple disciplines. Strong patient education and counseling skills. Excellent written and verbal communication abilities. Ability to prioritize responsibilities and respond effectively in urgent situations. Proficiency in electronic medical records and transplant data systems.
