RN Clinical Nurse Manager

About the Role
Soteria Home Health Agency is looking for a seasoned home health RN to join our in-office clinical leadership team. You’ll report to our Director of Nursing and work closely with our Operations and Compliance team. Your main focus will be clinical coordination, plan of care review, and keeping office operations running smoothly. This role is a great fit for an experienced home health nurse who knows the work from the field and wants to make sure every patient’s care is well coordinated, documented, and compliant.


Responsibilities

• Review and approve plans of care and updates for accuracy, medical necessity, and alignment with assessments and physician orders.
• Coordinate care with physicians, therapists, field clinicians, and other providers, including orders, referrals, and care conferences.
• Review OASIS assessments and clinical documentation for accuracy, timeliness, and compliance, and follow up with clinicians on corrections.
• Support the Director of Nursing with clinical oversight, staff guidance, and case reviews.
• Partner with Operations and Compliance on audit readiness, quality improvement, and corrective action plans.
• Support daily in-office operations, including intake, scheduling, and communication between the office and field staff.
• Help with staff training, in-services, and onboarding as needed.

Requirements

• Active California Registered Nurse (RN) license.
• At least 5 years of RN experience, including 3 or more years in home health.
• Strong experience with plans of care, OASIS, and home health documentation.
• Solid knowledge of Medicare and Medi-Cal home health rules and state regulations.
• Comfortable working in an EMR.
• Highly organized, detail-oriented, and a clear communicator.
• Supervisory or case management experience preferred.
• Valid driver’s license and reliable transportation for occasional field visits.
