POSITION SUMMARY

The Clinical Manager provides leadership, supervision, and support to an assigned clinical team and is responsible for the overall coordination and delivery of clinical services for a team of home health patients. This position works closely with clinicians, agency leadership, physicians, and other members of the interdisciplinary team to promote high-quality, patient-centered care.

WHAT YOU'LL DO

  • Provide day-to-day leadership, supervision, coaching, and support to a team of home health clinicians.
  • Coordinate the delivery of high-quality patient care while promoting positive clinical outcomes and an exceptional patient experience.
  • Assist in ensuring compliance with applicable federal, state, local, Medicare, and Medi-Cal regulations, as well as agency policies and procedures.
  • Review referrals and help ensure timely assessment and start-of-care visits.
  • Coordinate patient care assignments, clinical scheduling, staffing needs, and on-call coverage.
  • Conduct and facilitate interdisciplinary care conferences, team meetings, and clinical in-service education.
  • Monitor patient plans of care and collaborate with clinicians to address changes in condition, barriers to care, and patient needs.
  • Review clinical documentation and medical records to support quality, accuracy, completeness, and timeliness.
  • Provide ongoing coaching, education, and clinical guidance to field staff.
  • Serve as a clinical resource and preceptor for new and existing team members as needed.
  • Partner with agency leadership to identify opportunities to improve clinical quality, team performance, patient satisfaction, and operational effectiveness.
  • Foster a positive, collaborative culture focused on accountability, communication, teamwork, and exceptional patient care.

WHAT WE'RE LOOKING FOR

  • Graduate of an accredited school of nursing.
  • Current, unrestricted California Registered Nurse (RN) license.
  • Minimum of two years of clinical nursing experience within the last five years.
  • Home health experience strongly preferred.
  • Previous supervisory, clinical leadership, or management experience in home health preferred.
  • Strong understanding of home health clinical operations, patient care coordination, and regulatory requirements.
  • Excellent leadership, interpersonal, verbal, and written communication skills.
  • Ability to coach, mentor, and develop clinical team members while maintaining positive and productive working relationships.
  • Strong organizational skills with the ability to manage multiple priorities in a fast-paced clinical environment.
  • Valid driver’s license, reliable transportation, and automobile insurance.
  • Ability to make home visits and provide skilled nursing assessments and care in the home environment when needed.
