Duties<\/b>

• Provide clinical supervision and management of hospice care services, ensuring adherence to state and federal regulations, including Medicare guidelines.

• Develop, review, and update individualized care plans tailored to each resident’s needs, incorporating medical, psychological, and emotional support strategies.
  <\/li>
• Oversee documentation review, medical records management, and EMR (Electronic Medical Records) systems to maintain accurate, HIPAA -compliant records.
  <\/li>
• Lead interdisciplinary team meetings to coordinate care delivery, monitor resident progress, and adjust treatment plans as necessary.
  <\/li>
• Supervise nursing staff and other healthcare professionals, providing mentorship, performance evaluations, and ongoing training in nursing administration and shift management.

• Ensure compliance with healthcare laws such as HIPAA and maintain a thorough understanding of medical terminology, anatomy, physiology, and resident care standards.

• Manage day -to -day operations of hospice services within assisted living facilities, nursing homes, or home health settings while fostering a culture of compassionate care.

  Requirements<\/b>

  • Valid RN license with current state licensure required; experience in nursing home or assisted living environments preferred.

    <\/li>
  • Familiarity with EMR/EHR systems for medical records management and documentation accuracy.
