    East Georgia Registered Nurses - Bring Your Heart to Hospice Care

    Are you a Registered Nurse who believes great hospice care is about more than clinical treatment? Do you want to be a trusted advocate, guide, and source of comfort for patients and their families?

    Flexible Territory Options - Choose One

    We are hiring for two territory options:

    • Territory 1: Athens, Hall, Banks, and Habersham Counties

    • Territory 2: Clarke County

    Candidates will be considered for either territory based on their geographic area and patient needs.

    What You'll Do

    As a Hospice RN Case Manager, you will play a key role in ensuring patients receive exceptional, individualized care throughout their hospice journey.

    • Provide compassionate, skilled nursing care to hospice patients in their homes and other care settings

    • Complete comprehensive assessments and develop individualized plans of care

    • Monitor patient conditions and manage symptoms to promote comfort and quality of life

    • Collaborate closely with physicians, hospice clinicians, caregivers, and families

    • Educate and support patients and families throughout the hospice journey

    • Serve as a consistent clinical resource and advocate for your patients

    • Participate in interdisciplinary team meetings and coordinate patient care

    • Maintain accurate and timely clinical documentation

    What We're Looking For

    • Current RN license in the state of Georgia

    • Hospice, home health, palliative care, or related experience preferred

    • Strong clinical assessment and communication skills

    • Ability to work independently while collaborating with an interdisciplinary team

    • Compassionate, patient-centered approach

    • Valid driver's license, reliable transportation, and ability to travel within the assigned territory

    On An Average Day You Will:

    (includes, but not limited to)

    Make a true difference-one patient, one family, one compassionate moment at a time.

    • Develop and implement individualized care plans based on current and prior medical history

    • Deliver attentive, one-on-one support to patients and their families, ensuring comfort, dignity, and understanding

    • Conduct thorough assessments of physical, emotional, and spiritual needs throughout the care journey

    • Accurately monitor and document vital signs including temperature, pulse, respiration, and blood pressure

    • Administer physician-ordered medications and treatments with skill and empathy

    • Educate patients and their families on care techniques and symptom management

    • Track and communicate changes in patient condition, ensuring timely and effective responses

    • Keep detailed and up-to-date clinical notes to reflect patient progress and quality of care provided

    • Collaborate with interdisciplinary teams to ensure comprehensive care delivery

    • And contribute to other supportive duties as part of a compassionate, mission-driven team

    • And additional duties as assigned

    What Youll Bring to The Team:

    • A heart for hospice care and a commitment to exceptional service

    • A valid Registered Nurse license in good standing in the state you will work in

    • A valid driver's license and reliable transportation

    • CPR certification (current)

    • Strong observational and communication skills-both written and verbal

    • A flexible, team-oriented attitude and willingness to support additional duties as needed
