Salary: $83,000 – $110,000 per year
Schedule: Monday–Friday, 8:00 AM – 5:00 PM
UPTO $25K Sign On and up to $10K Relocation for qualified hire!
Job Description:
Seeking an RN Case Manager to coordinate patient care, improve health outcomes, and ensure cost-effective treatment plans. Responsibilities include managing acute and chronic cases, coordinating referrals, leading care plans with interdisciplinary teams, supporting patient education, and overseeing high-risk patient care.
Requirements:
- Active RN license (Alaska or eligible)
- BLS certification required
- 1–2+ years of nursing experience (case management or ambulatory preferred)
- Strong care coordination, communication, and clinical assessment skills
