What You'll Do as a Registered Nurse Case Manager

• Conduct in-person patient visits, interviews, and comprehensive assessments.

• Develop, implement, and continually evaluate individualized plans of care.

• Coordinate and oversee care provided by home health aides, LPNs, RNs, and other members of the care team.

• Communicate changes in patient conditions and collaborate with physicians and other healthcare providers to adjust care as needed.

• Educate patients and families on home health care, wound care, IV administration, medication management, and other aspects of their treatment.

• Promote preventative care, support continuity of care, and help reduce avoidable hospital readmissions.

What You'll Need

• Active RN license: Current, unrestricted Registered Nurse license in the state where you'll be practicing.

• Current certifications: CPR, AED, BLS, and First Aid, as required for the position.

• Reliable transportation to travel to patient homes and other work locations.

• Experience: At least one year of professional RN experience in home health or a similar setting.

• Previous case management or utilization review experience is preferred.

• OASIS experience is preferred.

• Tracheostomy and/or ventilator experience is a plus, but not required.
