We’re looking for a Home Health Case Manager who’s organized, clinically skilled,and ready to join a supportive team that values autonomy, teamwork, and work-lifebalance.What You'll Be Doing Manage the Care Journey: Serve as the primary point of contact for a caseload of homehealth patients, ensuring seamless coordination of services. Conduct Comprehensive Assessments: Complete OASIS assessments and developindividualized care plans in collaboration with the interdisciplinary team. Coordinate Services: Work with physicians, nurses, therapists, social workers, and homehealth aides to ensure timely delivery of care and optimal outcomes. Monitor and Evaluate: Track patient progress, update care plans, and communicate withfamilies and caregivers as needed. Documentation: Maintain accurate and timely clinical documentation in accordancewith Medicare guidelines and agency policy. Advocate for the Patient: Provide education, resources, and emotional support topatients and their families to promote independence and wellness.What We’re Looking For License: RN (Registered Nurse) with active(state) license. Experience: At least 1 year of experience in home health or case management preferred. Skills: Strong critical thinking, communication, time management, and the ability towork independently. Tech-Savvy: Comfortable using EHR systems (experience with Kantime, Axxess, HCHB,or WellSky is a plus).
