NEW RN/ Patient Care Coordinator (pediatric patients for the Pediatric Complex Care Program)
Required: - Active RN License
- BLS AHA certification
- Strong Current Peds experience
- Preferred: EPIC, Case Management
Patient Population: infants, children, and adolescents
****This role is hybrid, up to 3 in-person days and 2 remote (5) 8-hour days per week
No weekends, nights, on call, or holidays
Duties include helping coordinate care and clinically managing patients and caregivers. Patient education, reviewing after-visit summaries, managing EPIC in-basket, triaging calls, reviewing charts.
Will manage EPIC in-basket, triage calls, return messages to families, write letters, outreach to patients for barriers to care, work with DME, Nursing agencies, and other vendors, and review charts. Will not need to do med reconciliation or refills.
