RN Case Manager

Join our healthcare team to provide holistic care assessments addressing the multifaceted needs of patients. In this role, you will collaborate closely with patients, their families, physicians, and an interdisciplinary team to tailor and adapt personalized care plans throughout the patient care journey. Your hands-on nursing responsibilities will include administering advanced care therapies, managing wound and symptom care, and delivering vital patient and caregiver education on health management and self-care practices.

Your expertise will guide and supervise a dedicated team of licensed nurses and home health aides to ensure consistent, high-quality in-home care. As a key contributor to patient progress, you will adjust care strategies promptly and maintain thorough clinical documentation in line with organizational protocols.

Additionally, you’ll be actively involved in team meetings to drive continuous improvement in patient care and outcomes. This position requires efficient management of your caseload within Palm Coast, Florida, ensuring patient comfort and seamless care transitions. On-call participation and prompt response to emergent situations are also vital aspects of this role.

To qualify, you need an active RN license, experience in home health leadership, and proficiency with OASIS assessments—preferably with knowledge of HCHB systems. We are looking for a compassionate leader who can inspire and mentor a clinical team.
