RN Case Manager needed to provide Case Management to the developmentally disabled within OPWDD group homes.
20-40 hours a week depending on your availability with some flexibility in regard to the hours and days a week.
Job Duties
- Updating and maintaining the individuals monthly MARS and health records as required by OPWDD.
- Schedule and follow up on the individuals medical appointments.
- Request MD orders or extensions on existing orders or extensions when necessary.
- Be available to triage individuals if anything should arise while you are in the group home.
- Body checks/Assessments when needed as well as following up with nursing notes.
- Supervise med passes as needed for AMAP Certifications.
- OPWDD experience preferred.
$50-$53 an hour
