RN Case Manager

Pride Health is hiring an RN - Case Manager to support our client's healthcare facility in Milwaukee, WI.


Job Details:

• Schedule: Monday-Friday | 8:00 AM-5:00 PM | 
• Location: Milwaukee, WI; Travel between facilities required
• Job Type: Contract
• Contract Length: Approximately 4-6 Months 
• Pay Range: $40-$43

Pay offered is based on experience, expertise, credentialing, and education.

Duties:

• Ensure accurate and complete patient documentation, including assessments, medication reconciliation, nursing rounds, and coding.
• Communicate and collaborate with interdisciplinary care teams to coordinate patient care and support improved health outcomes.
• Assist providers and care teams with care coordination activities and Medicare Part A, Part B, and Part D benefit determinations.
• Collaborate with the primary advanced practice clinician (APC) and facility staff to identify and respond to changes in patient conditions.
• Coordinate therapy services and assist with managing patients during acute medical situations, chronic condition exacerbations, and short-term subacute rehabilitation.
• Participate in facility-based meetings, family conferences, team meetings, and case presentations.
• Coordinate training, track compliance with quality measures, and assist in developing and conducting in-service training sessions.

Requirements:
• License(s): Active RN license in the state of residence
• Certification(s): BLS
• Years of experience: 1+ year of clinical experience preferred; long-term care experience preferred
• Skills/Specialties/Technology: experience with geriatrics, chronic illness, and patients with complex behavioral, social, or functional needs.
