    We are seeking an RN Case Manager to join our Care Coordination team. This community-based nursing role helps patients overcome barriers to care, connect with needed resources, and improve health outcomes.     What You'll Do

    • Provide patient outreach, education, and care coordination in clinic, community, and patient home settings.
    • Assess barriers to care such as transportation, food access, housing, insurance, medications, and health literacy.
    • Connect patients with medical, behavioral health, social service, and community resources.
    • Collaborate with providers, care teams, social workers, and community partners.
    • Document outreach, referrals, care plans, and patient progress accurately and timely.

    What We're Looking For

    • Current nursing license in the state where services are provided; RN preferred.
    • Experience in healthcare, care coordination, case management, community outreach, or social services preferred.
    • BLS/CPR certification required or ability to obtain within required timeframe.
    • Strong communication, organization, documentation, and teamwork skills.
    • Valid driver's license, reliable transportation, and ability to travel locally as needed.
    • Bilingual English/Spanish skills preferred but not required.
