    Role Overview: The RN Case Manager (Inpatient Care Coordinator - ICC) plays a crucial role within the interdisciplinary team, overseeing patient care from admission to transfer or discharge. Their primary focus is optimizing resource utilization while ensuring effective discharge planning and patient advocacy.
    • Department: Case Management
      Shift: Monday to Friday, 08:00 16:30 (40 hours per week)
      Rotation: Includes rotating weekends
    • Experience Required: Minimum 2 years of recent acute care hospital discharge planning experience
    Key Responsibilities:
    • Care Coordination: Direct and coordinate patient care throughout their hospital stay, ensuring seamless transitions.
    • Comprehensive Assessments: Evaluate patients' healthcare and discharge needs, identifying potential challenges and aligning care plans with covered services.
    • Multidisciplinary Collaboration: Work closely with internal and external teams to develop safe and efficient discharge strategies that prevent readmissions.
