Interim HealthCare of Maine is hiring an RN Case Manager in Portland for a field-based home health role — one where you're behind the wheel, not behind a desk. You'll drive to patients' homes, build real relationships over the course of their care, and make clinical decisions alongside physicians as the primary point of contact for each case. If you're looking for more autonomy, variety, and one-on-one patient time than a facility job allows, this is that role. Compensation is $50.00/hour, paid weekly — among the strongest rates for home health case management in the region.
Responsibilities
- Conduct in-home visits to assess patient needs and physically evaluate condition per physician orders
- Travel within assigned territory to see patients on a scheduled basis
- Coordinate with healthcare providers, patients, and families to facilitate quality care.
- Monitor patient progress and adjust care plans as necessary.
- Provide education and support to patients and their families regarding care management.
- Document all case management activities accurately and timely in compliance with organizational policies.
- Collaborate with interdisciplinary teams to ensure comprehensive care delivery.
Requirements
- Current and valid RN license in the state of Maine.
- Experience in case management or related nursing role preferred.
- Strong assessment, organizational, and communication skills.
- Ability to work independently and collaboratively in a fast-paced environment.
- Proficiency with electronic health records and case management software is a plus.
- Valid driver's license and reliable transportation required
- Willingness to travel to patient homes within the territory
