You will supervise a team of LPNs, LVNs, and aides, ensuring all treatments are aligned with the highest standards of patient care. Maintain detailed electronic documentation of patient interactions and care plans, and actively engage in team meetings and quality improvement projects. Efficiently manage your caseload by scheduling visits in your territory and being responsive to patient needs, including participating in on-call duties.
Qualifications required include being a registered nurse with home health leadership and OASIS experience, ideally with HCHB knowledge. You will serve as the primary contact for field clinicians, offer support and mentorship, and oversee patients' episodes of care. Strong coordination and communication skills are essential to facilitate case meetings and utilization reviews. A bachelor's degree in nursing, one year of RN experience, and CPR certification are required. You must also possess a valid driver's license, an insured vehicle, and be prepared to travel to various locations as needed.
