RN Case Manager – Home Health

About the role

The RN Case Manager coordinates skilled home health care, completes assessments and OASIS documentation, develops and updates plans of care, and promotes safe patient outcomes.

What you will do

Provide and coordinate skilled nursing visits; complete timely assessments, OASIS, care plans, physician communication, patient education, medication review, and interdisciplinary case management; follow Medicare, HIPAA, infection-control, and agency standards.

Qualifications

Active Nevada RN license or valid compact privilege; current BLS/CPR; reliable transportation, driver’s license, and auto insurance for field duties. Home health and OASIS experience preferred. Employment is contingent on credential verification, background and exclusion screening, competency validation, and required health clearances.
