Responsibilities:
- Conduct comprehensive patient assessments to develop personalized care plans.
- Coordinate and manage patient care activities across interdisciplinary teams.
- Monitor patient progress and adjust care plans as needed to ensure optimal outcomes.
- Provide education and support to patients and their families regarding health management.
- Document all patient interactions and care plans in compliance with agency policies.
- Collaborate with physicians and other healthcare professionals to enhance patient care.
- Participate in quality improvement initiatives to enhance service delivery.
- Ensure adherence to regulatory requirements and best practices in home health care.
Requirements:
- Current RN license in the state of Indiana.
- Minimum of 2 years of nursing experience, preferably in home health or case management.
- Strong assessment, organizational, and communication skills.
- Ability to work independently and manage a diverse caseload.
- Experience with electronic health records (EHR) systems.
- Compassionate demeanor with a focus on patient-centered care.
- Basic Life Support (BLS) certification required.
- Knowledge of Medicare and Medicaid regulations is a plus.
