Job Responsibilities

Responsibilities

Conduct thorough patient assessments, including vital signs, medical history review, and physiological evaluations to determine care needs.
Develop and implement individualized care plans that align with clinical guidelines and patient goals.
Coordinate post-acute care services, including discharge planning and outpatient follow-up, ensuring continuity of care across settings.
Monitor patient progress through regular visits and telehealth check-ins, adjusting care plans as needed based on diagnostic evaluations and clinical findings.
Collaborate closely with physicians, therapists, social workers, and other healthcare professionals to optimize patient outcomes.
Manage medical records accurately while maintaining compliance with HIPAA regulations to protect patient privacy.
Educate patients and their families on disease management, medication adherence, and health promotion strategies.

Qualifications

Skills

Extensive critical care experience with a strong background in hospital medicine, emergency medicine, or ICU settings.
Experience working within Level I Trauma Centers or Level II Trauma Centers, demonstrating advanced trauma management skills.
Proven expertise in infectious disease care, post-acute care coordination, and discharge planning processes.
Knowledge of medical terminology, physiology, and diagnostic evaluation techniques essential for comprehensive patient assessment.
Skilled in utilization management, case management strategies, and managed care principles to ensure efficient resource use.
Familiarity with inpatient and outpatient clinical environments including pediatrics (PICU), geriatrics, behavioral health, and medical-surgical units.
Strong communication skills for effective intake procedures, medical records documentation, and interdisciplinary collaboration.
