    General Summary:
    Seeking a full-time candidate to work as an RN Case Manager. Responsibilities include performing post-discharge calls, case management evaluations, and acquiring services for discharge planning.
     
     Job Requirements:
    • Be proficient in performing post-discharge calls, to ensure that patients are continuing to do well post-
      discharge, and to answer any questions or concerns in regard to patients’ health
    • Assessing new patients by gathering information, reviewing diagnoses and analyzing medical test
      results
    • Developing complete plans of care to address the needs of patients, including (but not limited to)
      assessing patients who are at risk of adverse events, and at risk for re-admission
    • Educating patients about their care plan and health care options and helping them make key decisions
      about their health and post-discharge options
    • Scheduling appointments for doctors’ visits, therapies and other medical services
    • Revising care plans based on patients’ progress and input from physicians and other medical providers
    • Updating patient medical records after each contact
    • Offering emotional support for patients and their families
     Qualifications:
    •  Registered Nurse
    •  Available to work part-time
    •  Available to work some weekends
    •  Experience with Case Management (Preferred)
    Benefits:
    • Paid Sick Time - effective 90 days after employment
    • Paid Vacation Time - effective 90 days after employment
