Location: Apple Valley, CA
Job Type: Contract
Duration: 13 Weeks
Shift & Schedule: Day Shift | 8:00 AM – 4:30 PM | Monday–Friday | 40 Hours/Week | Every Other Weekend Based on Department Needs
Local Candidates Required: No
Traveler Candidates Required: Yes

Pay Rate: $2,770 – $2,880/week

Position Overview
We are seeking an experienced Registered Nurse/Case Manager to provide acute care case management, discharge planning, and care coordination services.

Required Qualifications

  • Active California RN license; pending license accepted if licensed by start date.

  • Minimum 2 years of acute care case management/discharge planning experience.

  • Hospital experience required.

  • Behavioral Health experience does not satisfy the required acute care experience.

  • Seasoned traveler required; first-time travelers are not accepted.

  • Ability to work every other weekend based on department needs.

Responsibilities

  • Provide case management services for acute care patients.

  • Coordinate discharge planning and transitions of care.

  • Assess patient needs and develop appropriate discharge plans.

  • Coordinate with physicians, nurses, social workers, patients, families, and other healthcare professionals.

  • Facilitate appropriate utilization of healthcare resources.

  • Assist with coordinating post-acute services and follow-up care.

  • Monitor patient progress and barriers to discharge.

  • Maintain accurate and timely case management documentation.

  • Support effective communication and continuity of care throughout hospitalization.

  • Follow hospital policies, regulatory requirements, and case management standards.

  • Perform other related duties as assigned.
