RN- Case Manager- Acute Clinical Care

Pride Health is hiring an RN - Case Management to support our client's healthcare facility in Altamonte Springs, FL. 

Job Details:

• Schedule: Monday-Friday | 8:00 AM-5:00 PM | 
• Location: Altamonte Springs, FL
• Job Type: Contract 
• Pay Range: $43-$46

*Pay offered is based on experience, expertise, credentialing, and education.

Duties: 

• Perform care management activities, including coordinating patient care and supporting appropriate healthcare services. 
• Conduct clinical assessments and telephonic triage, accurately document findings, and ensure appropriate follow-up. 
• Collaborate with physicians, patients, families, and ancillary staff to coordinate care and support effective discharge planning.
• Provide clinical guidance to non-licensed office staff and explain medical instructions to patients and their families.

Requirements:
License(s):Current Florida RN license 
Certification(s): BLS
Years of experience: Previous experience in an acute clinical care setting or equivalent; previous care management, utilization review, or discharge planning experience 
Skills/Specialties/Technology:  HMO experience preferred
