    Location:  Peoria, Princeton, and surrounding communities and counties.

    Position Summary

    Castor Home Nursing is seeking a compassionate and motivated Registered Nurse (RN) Care Manager to join our growing clinical team.

    This field-based position works with patients, families, nurses, and clinical leadership to support care coordination, clinical oversight, staff development, and quality patient care throughout Central Illinois.

    A company vehicle is provided for work-related travel.

    Responsibilities

    • Provide clinical oversight and care coordination for assigned patients.
    • Complete assessments, admissions, supervisory visits, and other clinical visits as needed.
    • Support and maintain strong relationships with patients, families, and field nurses.
    • Assist with nurse orientation, education, training, and competencies.
    • Monitor clinical documentation and plans of care.
    • Collaborate with clinical leadership and other members of the care team.
    • Support quality, compliance, and continuity of care.
    • Perform other clinical and administrative responsibilities as assigned.

    Qualifications

    • Active Illinois RN license required.
    • Pediatric, Private Duty Nursing, Home Nursing, Home Health, or care management experience preferred, but not required.
    • Strong communication, organization, and relationship-building skills.
    • Ability to work independently and manage multiple priorities.
    • Comfortable with a field-based position and regional travel.
    • Valid driver's license and acceptable driving record required.
