RN Care Coordinator

As a home care RN with New Dimensions, you will spend your days helping clients in the community served. Training provided. competitive wages and benefits offered!

Primary responsibilities:

• Managing medications
• Being the liaison between the client and their doctor
• Performing and documenting assessments
• Supervising PCAs
• Providing education to clients and families

Qualifications:

• Licensed as an RN with the Minnesota Board of Nursing
• Must be organized, have good assessment skills and the ability to work independently
• Must pass a background study
