    Description:

    Under the supervision of the Manager, provides coordination of care and patient education for patients in the in-patient and out-patient setting. Assists physicians in the provision of quality health care

    Responsibilities

    Job Requirement:

    Collects and assesses patient data to determine patient needs and to promote optimal care. Coordinate transfer of

    patient records. Calls and follows up with patients after surgery.

    Assist with follow-up on prior authorizations and other pending approvals for services.

    Assumes responsibility and accountability for the implementation of interventions based on the medical plan of

    care and patient's current condition.

    Plans and implements age appropriate care based on assessment, diagnosis, standard of care, medical treatment

    plan, and progress toward desired outcome. Assesses individual physical, psychosocial and developmental needs of

    patients.

    Coordinates education and guidance for the patient and significant others to maximize the achievement of the

    expected outcomes.

    Review and prioritize all EPIC in basket tasks and address urgent tasks as appropriate.

    Review and prioritize all faxes and address urgent faxes as appropriate.

    Link and provide support to other staff with issues related to Medicaid and other managed care plans, Agency of Persons

    with Disabilities, Home Health Agencies and durable medical equipment companies.

    Call patients who miss appointments to identify barriers and reschedule.

    Monitor patient ER/hospital admissions, follow patients while in hospital.

    Qualifications

    Qualification:

    Experience Requirements:

    Experience and/or an interest in working with youth and young adults ages 16-26 with special health care needs - preferred

    Experience with public benefits/social services, disability services and trauma informed care - preferred

    The ideal candidate would have a strong desire to work with underserved populations - preferred

    Education:

    Bachelors Nursing preferred

    Associate's degree required

    Certification / Licensure:

    Basic Life Support (BLS) required at time of hire; American Red Cross or American Heart Association
