    Job Title: RN Case Manager

    Location: Mission Viejo, CA 92691

    Contract length: 13 weeks

    Shift/Hours: Day 5x8-Hour (08:00 - 16:30)

    Job Description

    • Floating Expectation: Float to the other Facility

    • Ratios: 1:25

    • Years of experience REQ: 2-year acute medical Care Manager experience performing Care Coordination

    • Requires travel experience, HARDSTOP

    • Weekend REQ: Weekend Rotation

    • CA state license: Yes

    • RTO Restrictions: No more than 7 days
