Medicine That Meets Patients Where They Are

Healthcare can be complicated and overwhelming - especially for patients navigating multiple conditions, medications, and care settings.

At Riverside Home Health & Hospice, our Provider Services Nurse Practitioners help bridge that gap.

We bring medical care directly to patients - in their homes, assisted living communities, and skilled nursing facilities - helping them receive the care they need without unnecessary hospital visits.

If you're a Nurse Practitioner who enjoys autonomy, clinical decision-making, and building relationships with patients and care teams, this role offers the opportunity to practice medicine in a way that truly impacts lives.

• Diagnose and manage acute and chronic conditions
• Conduct patient assessments, prescribe medications, and guide treatment plans
• Collaborate with physicians, nurses, and community care teams
• Help patients receive the right care at the right time

Your work helps ensure patients experience timely, coordinated, high-quality care within their own communities.

Who Thrives in This Role

Our ideal candidate is a Nurse Practitioner who:

• Enjoys practicing independently while collaborating with physicians and teams
• Thinks critically and confidently in complex clinical situations
