This is an in-office role and not a telecommute or remote position.

Are you the One? We are looking for a Rural Health Transformation (RHT) Program Coordinator who will be responsible for the day-to-day management, coordination, and implementation of initiatives under South Carolina's RHT Program: Connections to Care, Leveling Up, Wellness Within Reach, Shoring up to Sustainability, and the Tech Catalyst Fund. This position ensures initiative-specific goals are met, subrecipients are supported and compliant, and measurable outcomes are achieved. The RHT program is a federal initiative led by CMS and focuses on promoting innovation, strategic partnerships, infrastructure development, and workforce investment to help state governments strengthen healthcare access, quality, and outcomes in rural communities. This position reports directly to the RHT Program Director within Health Programs

  • Coordinates the planning, implementation, monitoring, and evaluation of RHT-funded initiatives and pilot projects. Maintains and tracks timelines, deliverables, and reporting schedules to ensure the success of the RHT program and alignment with state and federal program requirements. Supports the development and management of subcontracts and MOUs. Conducts site visits to subrecipients twice per month.
  • Serves as the primary point of contact for initiative related stakeholders. Assists in the facilitation of stakeholder meetings, webinars, and community sessions to promote transparency, share updates, and collect feedback. Develops clear communication materials and reports for internal and external audiences.
  • Collects, reviews, and synthesizes data from subrecipients, in collaboration with RHT Data Specialist, to support program performance measurement. Prepares progress reports, budget updates, and dashboards for effective oversight. Assists with submission of CMS required reporting.
  • Supports the annual grant and review cycles. Serves as a subject matter expert providing knowledge, expertise, and deliverable/report reviews for assigned contracts. Assists with budget monitoring and invoice tracking for initiatives and subrecipients. Prepares materials for internal and external stakeholders (e.g., leadership briefings, legislative updates, and Governor's Office, etc.) reporting.
  • Leads or supports other projects proactively or as assigned by Agency leadership. Continuously engages in process improvement to support Agency and Division-wide strategies, objectives, and initiatives. Occasional travel may be required, as assigned.

Minimum and Additional Requirements

A Bachelor's degree in Public Health, Health Administration, Social Work, Public Policy, or related field and relevant professional experience in social service programs, business administration, general administrative management or relevant program experience.

Additional Requirements:

  • Occasional overnight travel.
  • Requires holder to drive routinely.
  • Overtime and/or weekend work with Deputy approval.

  • Sitting or standing for long periods of time.
  • Lifting requirements: 20 lbs.
  • A valid drivers license is required.

Preferred Qualifications

  • Three (3) years of experience in program coordination, project management, or grants administration-preferably in health, human services, or rural program settings.
  • Strong organizational and multitasking skills with attention to detail and deadlines.
  • Excellent written and verbal communication skills.
  • Proficiency in Microsoft Office Suite, SharePoint, and project management tools.
  • Familiarity with state or federal health programs, Medicaid, or community-based healthcare delivery preferred.
  • Ability to build and maintain collaborative working relationships with diverse partners.
  • Ability to travel statewide.
