The Registered Nurse (RN) will be responsible for providing care and services for a group of patients, visiting patients where they reside, which could include private homes, Skilled Nursing Facilities, and assisted living facilities. This position is a contributing member of the interdisciplinary team, providing expertise regarding psych-social assessments, interventions, and knowledge of community sources. The Registered Nurse (RN) will work in either a Resource role.

 

Candidate must be willing to cover all terrorities including Walnut Creek, Pinole, Oakland, Alameda, Solano County, Tri-Valley, Napa, Moraga, Martinez, Brentwood, etc.

Qualifications:
  • Current Registered Nurse (RN) license in good standing in the state of practice
  • A passion for providing exceptional, patient-centered hospice care
  • Strong collaboration and communication skills with interdisciplinary care teams
  • Ability to work independently while effectively managing your daily schedule
  • Excellent time management and organizational skills to balance patient visits, documentation, meetings, and urgent needs
  • Comfort learning and utilizing electronic medical record (EMR) systems
  • Reliable transportation, a valid driver's license, and current auto insurance to travel throughout the service area.
