Resource Registered Nurse (RN)

The Registered Nurse (RN) will be responsible for providing care and services for a group of patients, visiting patients where they reside, which could include private homes, Skilled Nursing Facilities, and assisted living facilities. The Registered Nurse (RN) will be responsible for providing care and services for a group of patients, visiting patients where they reside, which could include private homes, Skilled Nursing Facilities, and assisted living facilities. This position is a contributing member of the interdisciplinary team, providing expertise regarding psych-social assessments, interventions, and knowledge of community sources. The Registered Nurse (RN) will work in either a Resource role.

 

Candidate must be willing to cover all terrorities including Walnut Creek, Pinole, Oakland, Alameda, Solano County, Tri-Valley, Napa, Moraga, Martinez, Brentwood, etc.

Qualifications:

• Current Registered Nurse (RN) license in good standing in the state of practice
• A passion for providing exceptional, patient-centered hospice care
• Strong collaboration and communication skills with interdisciplinary care teams
• Ability to work independently while effectively managing your daily schedule
• Excellent time management and organizational skills to balance patient visits, documentation, meetings, and urgent needs
• Comfort learning and utilizing electronic medical record (EMR) systems
• Reliable transportation, a valid driver's license, and current auto insurance to travel throughout the service area.
