Summary:
Work Mode: Remote-GA, Field based
Location: Central region (Macon or closest proximity to Macon), SW region (Albany or Valdosta)
Target Start Date: 30 Nov 2026
Duration: 12 months with possibility to extend
Schedule: M-F, 8:30am - 5:30pm EST, No OT required
Responsibilities:
- Outreach, support, mentoring, education, and community linkages for women who have delivered Very Low Birth Weight babies.
- Educate and support members throughout their 12-month postpartum period.
- Improve the Plans postpartum visit HEDIS measure.
- Carry out the contractual requirements of the Planning for Healthy Babies Program.
- Ensure outreach metrics are completed in a timely manner, following SOPs and processes.
Requirements:
- High school diploma or equivalent required.
- 2 years of experience working with community agency providing services in health care, managed care, and member outreach.
- Driver’s license required.
Preferred Skills:
- Associate degree in related field preferred.
- Experience with home visits.
