    Coordinates and develops, reviews, and ensures implementation of ISP

    ● Coordinates and ensures the development of schedules, activities and routines and actively promotes opportunities for individuals to participate in community integrated activities, according to the principles of Person-Centered thinking.

    ● Oversees admissions and referrals

    ● Works collaboratively with other professionals involved in the individual’s services, such as behavior specialists, communication specialists or therapists, to ensure plans are implemented and progress documented.

    ● Coordinates and completes initial and on-going assessments of individuals.

    ● Develops and ensures implementation of planned program activities.

    ● Coordinates and serves as team leader in multidisciplinary team meetings pertaining to the individual’s service plan.

    ● Collaborates and cooperates with individuals, family members, other provider agencies, and advocates.

    ● Utilizes local resources to promote involvement in community activities, including volunteer opportunities.

    ● Develops educational and training materials, presents educational materials on various aspects of programs and provides assistance to staff related to Person-Centered Planning, ISPs, Outcomes and Self-Determination.

    ● Participates in interdisciplinary team meetings.

    ● Serves as a backup during emergencies

    ● Monitors compliance to all regulatory standards related to ISPs and programming.

    ● Reports and enters incidents as outlined in the Incident Management Bulletin and LNL’s policy on Incident Management.

    ● Assist in the development of data collection methods to report and monitor program activities and analyze data to determine if the activities provided are meeting program outcomes.

    ● Completes all assigned administrative tasks relative to the program.

    ● Participate in the company’s on-call system.

    ● Supervises direct support staff

    ● Provides overall on-site supervision and implementation of the daily operations of the home.

    ● Recommends and takes disciplinary action to ensure quality work performance of immediate staff.

    ● Ensures the individuals’ health, safety and welfare.

    ● Responds to individual’s complaints

    ● Ensures individual health and medical needs are met.

    ● Monitors and evaluates performance according to the company’s performance management policies and procedures.

    ● Monitors program and staff compliance to all company policies and procedures and all regulatory standards.

    ● Communicates relevant information to families, outside agencies, and other professionals as necessary.

    ● Oversees and maintains records pertaining to inventory, both for company owned property and possessions belonging to individuals, as applicable to the program.

    ● Accounts for all individual funds and petty cash funds; maintains an adequate supply of funds for the individuals and the home, as applicable to the program.

    ● Monitoring all non-reportable and reportable incident reports.

    ● Ensures all reportable incidents are finalized in EIM and corrective actions are followed carried out

    ● Investigating and following up on all incidents.

    ● Participates in the corporate decision-making process.

    ● Manages program and staff in compliance with company policies and procedures and all regulatory standards.

    ● Oversees incident management and follow-up.

    ● Oversees the investigation and resolution of program issues and grievances.

    ● Approves leaves, schedules personnel, and authorizes or assigns overtime.

    ● Reviews and evaluates the educational and training needs of staff and arranges for further training.

    ● Audits training and personnel records to ensure all staff are following regulatory requirements.

    ● Consults with program psychiatrist, or other delegated professionals, regarding clinical, medical and programmatic issues.

    ● Ensures appropriate coordination of services/ treatment and follow up.

    ● Oversees daily programs and or staff schedules.

    ● Oversees the development of data collection methods to report and monitor program activities, analyzes data to determine if the activities provided are meeting program goals.

    ● Operationalizes compliance with sanctioned philosophical and conceptual approaches.

    ● Participates in quality assurance/quality management (QM) activities related to the programs specified in the QM plan.

    ● Develops program policy and helps to coordinate it with overall company and program goals, directs the daily operations, evaluates results, and recommends changes in programs.

    ● Respond to emergency situations and provide leadership.
