What You''ll Do
Support Resident Well-Being
Guide Residents Through Life Transitions
Assist Residents and families in navigating significant life changes, including:
Chronic illness or declining health
Changes in mobility or activities of daily living
Loss of driving privileges and transportation planning
Home care arrangements
Temporary or permanent moves to Supportive Living
Hospitalizations
Loss of a spouse or loved one
Provide compassionate guidance and connect Residents with internal and external support resources.
Collaborate Across the Continuum of Care
Foster Positive Community Relationships
Coordinate Resident Transitions
