    Sonnet Hill Senior Living, located in San Jose, CA is seeking a dedicated and detail-oriented Resident Care Coordinator to provide, coordinate, and monitor high-quality care to the residents in our community.

    Responsibilities of the Resident Care Coordinator

    • Lead and support Care Partners and Med Techs; communicate professionally with residents, families, and healthcare providers.
    • Implement, update, and monitor individualized service plans and care documentation.
    • Conduct daily rounds to assess resident condition changes, verify appointments, and ensure staffing coverage.
    • Coordinate and monitor ADLs, care needs, and person‑centered support.
    • Document and report changes in resident condition; participate in service plan meetings.
    • Review medication exceptions, refusals, and unavailable meds; ensure proper documentation and follow‑up with the HWD.
    • Provide real‑time coaching and corrective guidance to care staff.
    • Monitor vital signs, hydration, diet orders, meal intake, and BM logs for accurate documentation.
    • Support new admissions: initiate care plans, coordinate physician orders, confirm medication delivery, and map residents in ALIS/Rx systems.
    • Review shower and skin documentation; follow the 3‑check system for bruises, skin tears, rashes, and wounds.
    • Conduct weekly ADL audits, medication room audits, and medication cart audits to maintain compliance and survey readiness.
    • Maintain supply levels and complete supply checklists.
    • Support infection control practices, PPE use, and hygiene standards.
    • Assist with monthly vital signs and weights, including reweighs and care plan updates.
    • Participate in new employee orientation, monthly in‑services, and ongoing staff training.


    Required Experience

    • Possess, at a minimum, a high school diploma or equivalent.
    • A minimum of two (2) years’ prior senior living experience preferred.
    • A minimum of one (1) year prior Caregiver and/or Med Tech experience preferred.
    • Be at least 18 years of age.
    • Basic computer skills.
    • Must be able to speak, read, and write in the English language.
    • State Criminal Background Check and LIC 508 Criminal Record Statement
    • LIC 503 Health Screening Report

    Job Type

    • Full Time
    • Schedule: Sunday through Thursday
