General Information:Job title: Resident Care CoordinatorSchedule: 80 hours per pay period; Monday - Friday - 8:00am to 4:30pmWeekend Requirement: No weekendsHoliday Requirement: Paid holidaysPosition Summary:The Resident Care Coordinator position assists with coordinating resident care through completion of the MDS, rounding with physicians, care planning, being available to direct care staff as a resource and providing help when needed, coordinating individual programs in order to help provide comprehensive resident care and gain accurate financial reimbursement to the facility.Position Responsibilities:Serve as the wound care coordinator.Coordinate resident care through completion of the MDS.Serve as the Restorative Program Coordinator.Oversee care plan meetings.Participate in physician rounds and assist direct care staff.Perform other duties as requested.Position Requirements:Two-year degree in nursing required.Must hold and maintain Wisconsin Registered Nursing license.Wound care certification, RAC certification preferred.MDS certification a plus.Current CPR certification.6+ months of related work experience required.Knowledge, Skills, & AbilitiesIntermediate proficiency with computers is required.Exceptional clinical skills.Strong oral and written communication skills.Self-starter with excellent interpersonal communication and problem-solving skills.Why Mile Bluff Medical Center?Mile Bluff Medical Center is a place where people come first.
