Pay Range:

  • LPN Base Pay: $40.00 - $45.00 per hour
  • RN Base Pay: $46.00 - $50.00 per hour

Shift Differential (to be added to base rate of pay):

  • Weekend Day Shift: +$2.75 per hour

Regular Schedule: Saturday and Sunday, 6:30am - 7:00pm


Horizon House is seeking a Part-time Resident Care Coordinator to join our Assisted Living team. In this role, you'll help ensure residents receive exceptional, person-centered care while serving as a key resource for residents, families, care providers, and direct care staff. You'll combine clinical expertise, care coordination, and team leadership to support a safe, engaging, and high-quality living environment.

What You'll Do

  • Coordinate and oversee resident care by conducting assessments, developing and updating care plans, monitoring resident well-being, and ensuring services meet residents' evolving needs.
  • Partner with residents, families, healthcare providers, and community resources to coordinate services, support care transitions, facilitate communication, and deliver a seamless care experience.
  • Provide leadership and support to direct care staff through coaching, training, competency oversight, and day-to-day guidance while fostering a collaborative, accountable, and resident-focused team culture.

What You'll Bring

Required Qualifications

  • Active Washington State LPN or RN license in good standing.
  • Current CPR certification or ability to obtain within 30 days of hire.
  • Food Handler's Permit or ability to obtain within 14 days of hire.
  • Proficiency with Microsoft Office applications, including Outlook, Teams, Word, and Excel.

Preferred Qualifications

  • Two or more years of nursing experience in Assisted Living, skilled nursing, rehabilitation, hospital, memory care, or related healthcare settings.
  • Experience completing resident assessments and developing care plans.
  • Knowledge of geriatric nursing, dementia care, and senior living services.
  • Experience mentoring, training, or supporting direct care team members.
