Unit Manager RN – Subacute Rehabilitation

Location: Voorhees
Job Type: Full-Time | Non-Exempt
Reports To: Director of Nursing
License Required: Active Registered Nurse (RN) License

Position Summary

We are seeking a dedicated and clinically strong Unit Manager RN to lead nursing care within our Subacute Rehabilitation Unit. This role combines hands-on clinical care with leadership responsibilities to ensure high-quality resident outcomes, regulatory compliance, and a positive, compassionate care environment.

The Unit Manager RN maintains 24-hour accountability for resident care and unit operations while collaborating closely with the interdisciplinary team to support resident recovery and well-being.

Key Responsibilities

Unit Leadership & Oversight

  • Maintain responsibility for the overall operation and clinical performance of the assigned subacute unit
  • Ensure appropriate staffing levels and verify daily staffing assignments
  • Conduct walking rounds and review 24-hour reports within two hours of arrival
  • Adjust schedule as needed to evaluate staff performance on all shifts

Resident Care & Clinical Excellence

  • Provide direct nursing care as required
  • Assess resident conditions, make clinical judgments, and notify physicians of changes
  • Monitor resident status changes and ensure timely communication with physicians and families
  • Participate in and lead resident care conferences
  • Ensure individualized care plans are effective and updated as needed
  • Monitor weights, behaviors, psychotropic medication use, and restorative nursing programs

Staff Supervision & Development

  • Serve as a role model and clinical leader for nursing staff
  • Provide guidance, coaching, and counseling to nursing team members
  • Participate in staff evaluations and performance management
  • Conduct periodic staff meetings to ensure communication, education, and quality improvement
  • Encourage professional growth and accountability among staff

Clinical Skills & Equipment Oversight

  • Maintain clinical competency with subacute-level equipment and procedures, including IV therapy, wound care, tube feedings, oxygen therapy, wound vacs, Foley catheter insertion, and emergency response
  • Ensure equipment is properly cleaned and functioning; report malfunctions promptly
  • Promote infection control and safety practices

Documentation & Compliance

  • Ensure accurate and timely clinical documentation by nursing staff
  • Monitor EMR documentation (Optimus/Kiosk) and monthly required reports
  • Oversee incident management, investigations, and reporting
  • Ensure compliance with Federal, State, OSHA, and facility regulations
  • Maintain resident rights, confidentiality, and ethical standards

Communication & Customer Service

  • Be accessible to residents and families for questions and concerns
  • Foster a compassionate, respectful, and resident-centered care environment
  • Collaborate effectively with therapy, dietary, administration, and other departments

Qualifications

Required

  • Active Registered Nurse (RN) license in good standing
  • Experience in subacute rehab, skilled nursing, or long-term care
  • Strong clinical judgment and leadership skills
  • Ability to handle high-pressure situations and prioritize resident safety
  • Excellent communication and interpersonal skills

Preferred

  • Prior Unit Manager, Charge Nurse, or supervisory experience
  • Experience with subacute rehab acuity and post-acute clinical care
